Technical Assistance:
Inspector is allowing center to obtain documentation within 10 business days to the verification of resilient surfacing requirments located under the toddler and school age playground equipment.

Comments:
An unannounced inspection has been conducted from 930-12. The Inspector observed the classes and the program. The Inspector checked staff records, special care provisions, menu, and daily attendance. Hand washing, toilet procedures, diaper changes, and supervision were also observed. Please call with questions 703-479-4679

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of inside and outside of the center, it was determined that the inside floors are not maintained in a clean manner and the outside preschool playground is not in a safe manner. Evidence: 1. Inside, the Inspector observed caked in dirt on the floors throughout the center and one bathroom has large rust stains evident on the floor as well as a dirty potty seat. 2. Outside, the Inspector noted 2 1 /2 inches of exposed metal posts under the equipment that should be covered under the ground. Also, the Inspector noted gaps between the seams that pose possible entangling of clothing and or hoodie strings.

Plan of Correction: We have placed bids for new cleaning staff. We have also contacted corporate about correcting the playground structure.

Standard #: 22VAC40-185-280-B
Description: Based on observation in the Preschool class, the center has not kept cleaning materials in an unlocked place. Evidence: 1. Cabinet above children's sink was unlocked and two cleaning bottles were in the cabinet.

Plan of Correction: Corrected on the spot and will remind staff to keep locked.

Standard #: 22VAC40-185-280-D
Description: Based on observation, the center has not kept cleaning materials stored in areas separate from single-service utensils and articles. Evidence: 1. Cleaning bottles observed in the preschool cabinet were stored in direct proximity over containers holding single use utensils, bowls, and napkins.

Plan of Correction: Corrected during the inspection by removing items.

Standard #: 22VAC40-185-350-C
Description: Based on observation and staff interviews, the ratio in the toddler room does not meet requirements when a transition of a younger child occurs. Evidence: 1. One staff member was present with 5 children in the toddler area. The Inspector asked for the age of the youngest child and was told child A was 15 months old. The staff had said the child is in transition to become a toddler but did not realize the ratio would be lowered to 4 to 1. 2. Staff in the Infant room stated parent was signing off but that they did not conduct the assessment to verify child was ready for 5 to 1 ratio.

Plan of Correction: This was a misunderstanding about transition and we will correct the process to be sure the ratio is followed.
