Comments:
An unannounced non-mandated monitoring inspection was completed on 11/30/2017. There were 22 children, ages 6 months-5 years, and 5 staff present during today's inspection. The inspector reviewed 5 children's records and 5 staff records. This facility does not administer medication. The toddlers and preschool-aged children were observed in the following activities: finishing up lunch, during a bathroom break and hand washing, and nap time. The infants were observed in free-choice activities and being fed and allowed to sleep on demand. The inspector reviewed the violations from the previous inspection. The inspector discussed the following with the staff: tracking immunization records for infants and toddlers, following the centers Termination Policy regarding behavior, storing personal use items, and safe sleep procedures. An exit conference took place at the conclusion of today's inspection. The inspector arrived at 11:50 a.m. and departed at 2:10 p.m. Thank you for your time today. Violations:
Standard #: 22VAC40-185-440-N
Description: Based on observations, the facility failed to ensure that cribs with infants over the age of 5 months do not have toys or objects strung across the crib. Evidence: The crib for Child #1 had a chain of plastic break-away link toys strung across the crib; Child #1 is 6 months old.

Plan of Correction: The chain of plastic break-away link toys was removed during the inspection.

Standard #: 22VAC40-191-60-C-1
Description: Based on record review, the facility failed to ensure that all employees have an original criminal history record report within 30 days of employment. Evidence: 1. The record for Staff #4 has a documented hire date of 9/25/2017; the completed criminal history record report is dated 11/02/2017. 2. The record for Staff #5 has a documented hire date of 9/25/2017; the completed criminal history record report is dated 11/03/2017.

Plan of Correction: Criminal history requests will be sent within 7 days of hire.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the facility failed to ensure that all employees have an original search of the central registry within 30 days of employment. Evidence: 1. The record for Staff #4 has a documented hire date of 9/25/2017; the completed search of the central registry was dated 11/08/2017. 2. The record for Staff #5 has a documented hire date of 9/25/2017; the completed search of the central registry was dated 11/12/2017.

Plan of Correction: Search of the central registry findings will be requested within 7 days.
