A Subsidy Health and Safety inspection was conducted on 11/28/2017 to review supplemental health and safety requirements. The inspection began at 11:30am and ended at 1:30pm. There were 22 children: 4 infants with one staff, 5 toddlers with one staff and thirteen preschoolers with two staff. The children were observed dancing to music, during lunch and during naps. Five staff?s records and five children?s records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Standard #: VENDSUB-000-012-B Description: Based on record reviews, the vendor failed to obtain documentation of Virginia Preservice Training for staff within 90 days of employment. Evidence: 1. The inspector reviewed five staff's records. One of five records reviewed (staff record #4) did not contain documentation of preservice training and staff has been employed for more than 90 days. Staff#4's date of employment was 3/6/17. Plan of Correction: Staff #4 will complete the preservice training and provide documentation for their records by 12/12/2017.
