22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal study was conducted from 9:20 a.m. to 1:20 p.m. with the director. 57 children ages four months through five years old were under the care and supervision of 12 staff. Children were observed participating in morning circle, working on a letter art activity, putting together puzzles, playing outside and eating lunch. Lunch served today was fish sticks, whole wheat bread, apple slices, corn and milk. Required information was posted. Both the fire and health inspections are current. A complete first aid kit, battery operated radio, and battery operated flashlight were present. Three outdoor play areas and one bus were inspected. There were no medications to review. Five children's records, five staff records, and 22 injury reports were reviewed. A subsidy health and safety inspection (SHSI) was also conducted. Standard #: 22VAC40-185-150-B Description: Based on review of five children's records, the center did not have immunization documentation that was signed or stamped by a doctor, his designee, or local health department official. Evidence: Child A's immunization documentation dated 3/15/17 was not signed or stamped. Plan of Correction: Future enrolled children's health documentation will be reviewed more closely for signature or stamp.
Standard #: 22VAC40-185-160-A Description: Based on review of five staff records, the center did not have documentation of a tuberculosis (TB) test or screening for staff within 21 days of hire or within 12 months prior to hire. Evidence: Staff A's (hired 8/29/17) TB test was dated 10/8/17 and Staff B's(hired 9/12/17) TB test was dated 8/25/16. Plan of Correction: The director will set a reminder about TB test date time lines on the calendar.
Standard #: 22VAC40-185-580-B Description: Based on observation, the vehicle used by the center for the transportation of children does not meet the safety standards set by the Department of Motor Vehicles. Evidence: Bus 1124 had a safety inspection rejection sticker. Plan of Correction: Corporate maintenance already addressed the issues, and the bus will be taken back for re-inspection.
