Comments:
An unannounced monitoring inspection was conducted on December 1, 2017. Upon arrival to the facility, there were 20 children present with five staff members. Additional staff were on site for administrative and support purposes. The following items were reviewed as a part of the inspection: indoor and outdoor areas, supervision, administration, health and safety, physical plant, required documentation, and emergency supplies, procedures and required postings. The center will be administering medication however there was no medication present in the center at this time. The center does offer transportation services.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child shall have a physical examination by or under the direction of a physician: Before the child's attendance; or within one month after attendance. Evidence: During the inspection conducted on December 1, 2017, there was no documentation of a physical examination in the record for Child #5 (start date 10/30/17).

Plan of Correction: Owner spoke with the child's parent and a copy will be provided next week.

Standard #: 22VAC40-185-150-B
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each medical report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. Evidence: During the inspection conducted on December 1, 2017, the immunizations for Child #2 had not been signed by a physician, his designee, or an official of a local health department.

Plan of Correction: Owner will contact the child's parent to obtain a signed copy.

Standard #: 22VAC40-185-550-D
Description: Based on review, the licensee did not ensure that the center would implement a monthly practice evacuation drill for the most likely to occur scenarios. Evidence: During the inspection conducted on December 1, 2017, there was no documentation of an evacuation drill practiced in November 2017.

Plan of Correction: We will conduct a fire drill.
