Comments:
An unannounced monitoring inspection was conducted at the facility on December 5, 2017 from approximately 10:15am- 12:30 pm. Upon entrance there were forty children with 9 staff. Additional staff was present during the inspection. The licensing inspector observed the children freely playing, napping and outside play. The sample size consisted of 5 children records and 5 staff records. Areas of non-compliance was reviewed with the director. Medication is not given at the center. Exit interview was conducted with the program director. The licensee must send documentation to the LI that the background checks have been requested no later than 10 days following this notification.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure the diapering surface was used only for diapering or cleaning children and the diapering surface was a nonabsorbent surface for diapering. Evidence: During the inspection conducted on June 9, 2017,the following was observed: -In the Lady Bug classroom, there were nap mats on top of the diapering surface. -In the Caterpillar classroom there was a tear on the diapering pad.

Plan of Correction: The program director will replace the diapering pad in the caterpillar classroom being replaced. The program director will speak to the teachers.

Description: Based on record review, the licensee did not ensure the staff completed sworn statement or affirmation. Evidence: During the inspection conducted on December 5, 2017, the center did not use the updated sworn statement or affirmation form. The form was updated July 1, 2017. Staff #2 ( hire date July 10, 2017) , staff #3 ( hire date September 5, 2017), staff #4 ( hire date September 5, 2017) and staff #5 ( hire date August 24, 2017) did not have the updated sworn statement or affirmation form completed.

Plan of Correction: The program director has copied the new form and staff will fill it out.
