Technical Assistance:
Reviewed all current and upcoming changes to background check procedures. All future information regarding background check procedures will only be distributed via email.

Comments:
An unannounced monitoring inspection was conducted today. There were 86 children in care, with a total of 12 staff members assigned to their direct supervision. Children's records, staff records, staff training documentation, the indoor and outdoor physical plant, emergency preparedness items, and children's medications were inspected today. The children were observed creating Christmas presents, making evergreen collages, playing candy cane counting games, playing outside, creating child directed classroom art, and listening to a guest reader presentation. This inspection was completed between approximately 9:45am and 11:40 am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #4 does not include documentation showing that a staff person has viewed the child's original proof of identification.

Plan of Correction: I will follow up with the parent to bring the child's birth certificate.

Standard #: 22VAC40-185-350-E-4
Description: Based on observations, it was determined that one class was briefly operated beyond the allowable staff:child ratio of 1:10. Evidence: The Inspector observed 15 children in care in the Pre-K classroom, with only one staff person present in the classroom. The second staff person was out of the classroom for approximately 2 - 3 minutes.

Plan of Correction: The second staff person stepped out briefly to use the restroom. All staff will be reminded to call for a replacement staff person if one staff needs to step out of the classroom.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that one authorization form has expired. Evidence: The authorization form for one child's nonprescription medication expired on 9/6/17.

Plan of Correction: The medication is no longer needed. I will return the medication to the parent.
