Standard #: 22VAC40-185-60-A
Description: Based on 11 children's records reviewed, children's records were incomplete. Evidence: 11 of 11 records reviewed were incomplete. 1) Child A was missing the mother's work phone number, father's employer and employer phone number, and the addresses of 2 of 2 emergency contacts. 2) Child B, C and D were missing the addresses of 2 of 2 emergency contacts. 3) Child E was missing the addresses of 2 of 2 emergency contacts, employment information of both parents, physician's name and phone number, and proof of identification. 4) Child F was missing the fathers home address, employment information of both parents, and 1 of 2 emergency contacts. 5) Child G was missing the father's work phone number, and the addresses of 2 of 2 emergency contacts. 6) Child H was missing the mother's employment information, and the addresses of 2 of 2 emergency contacts. 7) Child I was missing the father's employment information. 8) Child J was missing the work phone numbers of both parents. 9) Child K was missing the employment information of the mother, the fathers work phone number, and 1 of 2 emergency contacts.

Description: Based on observation, the playground was not maintained in a safe condition. Evidence: The rubber was peeling on first step leading up to the slide on the main playing equipment. The metal was exposed and caused a finger entrapment hazard.

Plan of Correction: The maintenance manager was notified and a work order will be submitted.

Standard #: 22VAC40-185-550-M
Description: Based on the review of 6 injury logs, injury logs were missing required information. Evidence: 1) 2 were missing the future action. 2) 1 was missing the time the parent was notified. 3) 1 was missing the date the parent was notified.

Plan of Correction: A new form going to be used, staff will be trained on how to complete the new form.

Description: Based on observation, the most recent inspection was not posted. Evidence: The violation notice that was posted was dated 2016 and the last inspection was conducted 5/16/2017.
