Discussion was held on the topics of current enrollment, current staffing, the new definition of a serious injury, and a review of changes to background checks with regards to expiration dates, new sworn disclosure statements, and to obtain CPS checks from other states as listed from the past five years.
The following inspection took place between the hours of 10:30 am through 3 pm. Today, I met with the Staff of the Goddard School of Ashburn located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 94 children with 16 classroom staff; good ratios in all 9 rooms. A total of 9 children?s files and 5 staff files were reviewed. The children were engaged in the following: story time, holiday crafts, preparing for tonight's holiday program, games, diaper changing, hand washing, bathroom breaks, infant feeding, centers within the room, song, music, lunch, nap time, and other organized activities. Standard #: 22VAC40-185-60-A Description: Based on review, 2 of 9 children's files reviewed indicated that not all of the required information was contained within each child's record. Evidence - 1. On the date of inspection (12/08/2017), the addresses for each emergency contact was not available for review for the following children: Child #1, and Child #9. 2. The name and phone numbers were listed. The completed address for both were left blank. Plan of Correction: All missing information shall be documented and placed on file.
Standard #: 22VAC40-185-420-E-3 Description: Based on review, 6 of 9 children's files reviewed indicated that staff did not obtain (at least annually) parent confirmation that the required information in their child's record was up to date; and that such sharing of information was documented. Evidence - 1. On the date of inspection (12/08/2017), the following children had review documentation older than one year: Child #1, Child #2, Child #5, Child #6, Child #8, and Child #9. 2. The most recent documentation was listed as - Child #1 (07/22/2014), Child #2 (02/18/2016), Child #5 (08/21/2015), Child #6 (10/22/2016), Child #8 (03/18/2016), and Child #9 (05/14/2014). Plan of Correction: A undated form shall be sent out to all applicable parents and placed on file when completed.
Standard #: 22VAC40-191-40-D-1-C Description: Based on review, 1 of 5 staff files reviewed indicated that it has been more than three (3) years since the date of the most recent criminal history record check (CRC) report. Evidence - 1. On the date of inspection (12/08/2017), a current CRC was not available for Staff #2. 2. The most current CRC for Staff #2 was dated - 08/04/2014. Plan of Correction: A request for an updated CRC shall be sent to the Virginia Central Criminal Records Exchange (Virginia State Police) within the next 10 days. Proof that this request was sent shall be submitted to the Licensing Inspector (LI). Once this finding has been obtained, the LI shall be contacted for its review.
Standard #: 63.2-1721.1-B-3 Description: Based on review, 1 of 5 staff files reviewed indicated that the center did not obtain a search of the central registry (CPS) maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him or her. Evidence - 1. On the date of inspection (12/08/2017), a Virginia search of the central registry conducted on Staff #3 indicated that this staff person resided within the State of Maryland up to August 2016. 2. A CPS check from the State of Maryland for Staff #3 was not available for review. Plan of Correction: A CPS request shall be sent to the State of Maryland within the next 10 days. Proof that this request was sent shall be submitted to the Licensing Inspector (LI). Once this finding has been obtained, the LI shall be contacted for its review.
