Comments:
An unannounced monitoring inspection was completed on December 8, 2017. The purpose of this visit was to measure compliance with the licensing standards. There were 50 children, infant to preschool age, in care at the time of the inspection. There were 11 staff members working directly with the children in their classrooms. All classrooms maintained the proper ratio. The children throughout the facility were eating lunch. For lunch, the children were served pizza on whole grain crust, peas and carrots, pineapples and milk. Staff was observed interacting with the children and providing assistance when needed. The facility was equipped with manipulatives and supplies and items were available to the children. Medication is administered and multiple staff has MAT training. All staff have cpr/first aid training. Last documented health inspection: 5/19/17 Last documented fire inspection: 2/7/17 During the inspection, 5 children's records and five staff records were reviewed. Approximate time on site: 11:30am to 2:00pm

Violations:
Standard #: 22VAC40-185-150-B
Description: Based on a review of a sample of children's records, the facility did not ensure that a child's immunization and physical exam form was signed by a physician , his designee or a health department official. EVIDENCE: The physical and immunization records for child#5, were not signed by a physician.

Plan of Correction: I will contact mom today and tell her that we need them with the physician's signature.

Standard #: 22VAC40-185-60-A
Description: Based on a sample of children's records, the facility did not ensure that two children's records contained addresses for both of the emergency contacts. EVIDENCE: The records of child #1 and child#2, did not contain addresses for both of the emergency contacts.

Plan of Correction: We sent home new forms for the parents to complete in full.

Standard #: 22VAC40-185-270-A
Description: Based on observations in the facility, the facility did not ensure that all areas and equipment were maintained in a safe and operable condition. EVIDENCE: The inspector observed display boards on the wall in the toddler room, that were accessible to the children, that had broken Plexiglas. One smaller piece had a jagged edge that presented a hazard.

Plan of Correction: We removed the broken glass and we will order new plexiglass.

Standard #: 22VAC40-185-280-B
Description: Based on observations at the facility, the facility did not ensure that a hazardous substance was kept in a locked place. EVIDENCE: The inspector observed a small tub of spackle sitting on the bench outside of the infant classroom.

Plan of Correction: It was put away.
