Comments:
An unannounced monitoring inspection was conducted from 8:49am-12:15pm. At the time of entrance, 34 children were in care with 9 staff members present. Children were observed during snack time, playing with table toys, engaged in group activities and circle time. A selection of staff and children records, medications and other documents were reviewed. The center is clean and organized. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain a safe and operable condition of equipment outside. Evidence: 1. A red riding toy located on the playground had two damaged tires with holes and cracks on each tire.

Plan of Correction: Removed the red riding toy during the licensing visit and threw away. Retrained teachers on inspection of playground equipment.
