Comments:
An unannounced monitoring inspection was conducted from 10:35 am-1:40pm. There were 42 children were in direct care of 15 saff members. Children were observed returning from Christmas caroling, using the bathroom, washing hands, playing on the playground and singing songs. The center has an ample supply of books, table top toys and center manipulatives for the children to use. Staff to children interaction was observed to be positive. A selection of staff and children records, and other documents were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and staff interview, staff did not submit documentation of a negative Tuberculosis (TB) screening within 21 days of starting employment. Evidence: 1. Staff # 1 was hired on 11/8/17. 2. Center director stated that Staff # 1 had not obtained the documentation for a negative TB screening and it was not in the staff member's file.

Plan of Correction: Staff will get the TB screening done.

Standard #: 22VAC40-185-240-C
Description: Based on record review, staff working directly with children did not maintain 16 hours of annual training. Evidence: 1. Staff # 2 was hired on 10/7/13. 2. Staff # 2's records did not include any training documentation since 11/3/15. 3. Center director stated that Staff # 2 recently got some training, but could not recall exact dates and documentation could not be obtained during the inspection.

Plan of Correction: We will make sure that the training hours be completed and documentation added to the file.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the diaper changing pad was not non-absorbent and a foot pedal system was not in place for the disposal of soiled diapers. Evidence: 1. The diaper changing pad for the Older 2's classroom had a large tear making it an absorbent surface. 2. The diaper disposal system (diaper pail) in the bathroom used by the Older 2's classroom was broken. The foot pedal was inoperable as it was broken. 3. Staff # 4 stated that they use this diaper pail for children that are getting potty trained, to dispose of their wet diapers.

Plan of Correction: The diaper pad will be changed. The diaper disposal system will be replaced.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on record review and staff interview, center did not obtain a criminal history check for staff within three years of last check. Evidence: 1. Staff # 3's hire date was 7/2012. 2. The date of Staff # 3's most recent criminal history check was 3/2013 and expired in 3/2016. 3. The center mailed the paperwork to update the criminal history check on 11/8/17. The criminal history check should have been updated in 2016.

Plan of Correction: We have put the search in the mail.

Standard #: 22VAC40-191-60-B
Description: Based on record review, center employed a new employee without completing the new sworn statement (SS). Evidence: 1. Staff # 1's hire date was 11/8/17. 2. Staff # 1 did not complete the new SS that went into effect as of July 1, 2017.

Plan of Correction: We will have staff fill out the updated form.

Standard #: 63.2-1720.1-B-3
Description: Based on record review, center did not obtain an out of state central registry for staff that has resided outside of Virginia within the last 5 years. Evidence: 1. Staff # 2's records indicated that she resided outside of Virginia in the last five years. 2. An out of state central registry was not completed for this staff member.

Plan of Correction: We will submit the out of state central registry for the staff member.
