Today we discussed changes background check regulations and compliance, serious injury reporting and documentation of annual children's records update.
An unannounced monitoring inspection was conducted between 12:50pm and 4:00pm. There were 62 children ages 2months through five years present for care with 14 teaching staff the program director and assistant program director. Children and staff records were reviewed. Children were observed during nap, transitions, inside and outside play activities, restroom routines and hand washing. Required postings, inspections and emergency drills were reviewed. Violations regarding emergency supplies, diaper changing, daily communication with parents and background checks were cited and appear on the violation notice.
Standard #: 22VAC40-185-420-E-1 Description: Based on record review and interview the center failed to ensure that the daily record for each infant contain all the required information. Evidence: 1 - The daily record for two month old child 1 on 12/6/17 does not contain documentation of tummy time or the amount time the infant slept. 2 - Staff 1 confirmed that the daily for child 1 did not contain documentation of tummy time or the amount of sleep for the infant. Plan of Correction: Center management will remind and retrain infant room staff on the need to fill out the daily sheets completely for all infants, each day.
Standard #: 22VAC40-185-500-B Description: Base on observation and interview the center failed to ensure that diapering surface is cleaned with soap and at least room temperature water and sanitized after each use. Evidence: 1 - Staff 6 changed the diapers of three children in a row without cleaning and sanitizing the diaper surface. a. Staff 6 sprayed the diaper changing surface with a solution from a bottle labeled sanitizer then immediately wiped the surface dry. 2 - Staff 6 stated that she had not both cleaned and sanitized the diapering surface between each diaper change. Plan of Correction: Center management will retrain all staff in proper cleaning and sanitation of the diaper changing surface.
Standard #: 22VAC40-185-570-I Description: Based on observation and interview the center failed to ensure that a one-day's supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care shall be maintained at the center. 1 - Staff 3,4 and 5 stated that they were unaware of an emergency supply of disposable bottles and commercial formulas appropriate for the children in care. 2 - Staff 1 stated that she was not certain that the formula available as the emergency supply was appropriate for all infants enrolled. 3 - Staff 1 confirmed that there was not a supply of disposable bottles in the center. Plan of Correction: Center management will ensure that all required emergency infant supplies are maintained in the center and the staff are aware of the location of the supplies.
Standard #: 22VAC40-191-60-C-1 Description: Based on record review and interview the center does not have an original criminal history record report within 30 days of employment. Evidence: 1 - The record for staff 2 has a documented date of hire of 10/2/17. However there is no documentation of a completed criminal history record report in the file. 2 - Staff 1 confirmed that a completed criminal history record report was not present in the record of staff 2. Plan of Correction: Within 10 days of this inspection center management will send documentation of the criminal history report request to the department.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and interview the center does not have an original central registry finding within 30 days of employment. Evidence: 1 - The record for staff 2 has a documented date of hire of 10/2/17. However there is no documentation of a completed central registry finding in the file. 2 - Staff 1 confirmed that a completed central registry finding was not present in the record of staff 2. Plan of Correction: Within 10 days of this inspection center management will send documentation of the criminal history report request to the department.
