Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-185-(7)-500-A (Hand washing); 22VAC40-185-(7)-500-B (Diaper changing)

Comments:
An unannounced monitoring inspection was conducted on 12/11/17 from 10:02am - 12:30pm. During the inspection there were 39 children ages five months old to four years old in care with 9 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, eating lunch and during rest period. Records were reviewed for five children and five staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on a review of five children's records, it was determined that the facility did not ensure that they obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #1 (date of birth 9/29/15) contained an immunization record that was dated 1/3/17. 2. The record for child #1 (date of birth 3/5/16) contained an immunization record that was dated 6/1/17. 3. Staff #1 (Program Director) reviewed the record for child #1 and child #3, and confirmed that an updated immunization had not been received within 6 months.

Plan of Correction: The facility responded: The parents of child #1 and child #3 will be asked to provide an updated immunization. Center management will review the records of all children under the age of two years old to ensure they have an updated immunization record.

Standard #: 63.2-1720.1-B-3
Description: Based on a review of five staff records the facility did not deny continued employment of a staff who did not have the Out of State Child Abuse and Neglect Results by the end of the 30th day of employment. Evidence: 1. The record for staff #5 (date of hire 8/9/17) did not have documentation for the Out of State Child Abuse and Neglect Results. 2. Staff #1 (Program Director)) reviewed the record for staff #5, and confirmed that the Out of State Child Abuse and Neglect Results had not been received within 30 days.

Plan of Correction: The facility responded: We will complete the Out of State Child Abuse and Neglect Results for staff #5. All new employees who have lived outside the state of Virginia will complete the Out of State Child Abuse and Neglect Results.
