22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from approximately10:40am-12:40pm. There were 56 children in care directly supervised by 14 staff. A sample of 6 children?s records and 5 staff records were reviewed. The children were observed painting, playing on the playground, during bathroom procedures and reading. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Standard #: 22VAC40-185-60-A Description: Based on records reviewed, children's records were incomplete. Evidence: 2 of 6 children's records reviewed were incomplete. 1) Child A and B were missing the father's employer phone number. Plan of Correction: Parents use their cell phones. Files will be updated.
Standard #: 22VAC40-185-80-A Description: Based on observation, a written record of daily attendance was not kept. Evidence: "Room 7" did not keep a written records of daily attendance for the children in care. There was no evidence that attendance had been completed for the previous weeks. Plan of Correction: Teacher will start a record of attendance today.
Standard #: 22VAC40-185-280-B Description: Based on observation, cleaning materials were not kept in a locked place. Evidence: In "Room 5" all purpose wipes were located on a counter near the entrance of the classroom. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-185-280-D Description: Based on observation, cleaning materials were not kept separate from food equipment and utensils. Evidence: 1) In "Room 2" a bottle labeled "bleach" was found in a cabinet about plated, forks and cups. 2) In "Room 6" a bottle labeled "bleach" was found in a cabinet about plated, forks and cups.
