Standard #: 22VAC40-185-500-B Description: Based on review of the physical plant, it was determined that the facility did not ensure that the diapering surface must be nonabsorbent. Evidence: The diaper changing pad in the infant area had tears in it which did not allow the surface to be nonabsorbent. Plan of Correction: Diaper pad was replaced during the inspection.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of 14 staff records, it was determined that the facility did not ensure that staff who did not have an original completed criminal history record on file within 30 days of employment were denied continued employment. Evidence: 1. Staff #13, date of employment 10/23/17, did not have an original criminal history record report on file. Center management indicated that staff #13 was on the schedule at the facility. 2. Staff #14, date of employment 9/8/17, did not have an original criminal history record report on file. Center management indicated that staff #14 was on the schedule at the facility. Plan of Correction: Facility must provide to the department with documentation that the criminal record checks have been submitted to the Virginia State Police within 10 business days. Completed checks must also be submitted to the department once they are returned from the Virginia State Police.
