22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today beginning at 11:40am. 32 children were directly supervised by 10 staff members. Ratios were in compliance. Required documents were posted. The physical space, attendance records, 5 children's records, 5 staff records, evacuation drills, injury reports, the menu and medications were reviewed. Today's lunch was baked ziti, salad, fruit and milk. Staff were actively engaged with the children and overall record keeping was in good order. Standard #: 22VAC40-185-160-C Description: Based on review of 5 staff records, all staff members did not submit subsequent tuberculosis screenings within 2 years after the initial screening. Evidence: The TB screening for Staff #1 expired on 11/30/2017. Plan of Correction: I will send the teacher to get the screening right away.
Standard #: 22VAC40-185-50-A Description: Based on observation, children's records were not treated confidentially. Evidence: Allergy lists, with children's names & photos, were posted in the classrooms without a cover page to protect information. Plan of Correction: We will make sure information is always treated confidentially. The director placed cover pages on all of the lists while the LI was present.
Standard #: 22VAC40-185-270-A Description: Based on inspection, one of bathrooms and one of the classrooms were not maintained in a safe and operable condition. Evidence: The tile floor is cracked and raised around the drain, in the bathroom across from the Toddler room. There is a hole in the wall (the size of the doorknob) behind the door in the School Age room. In addition, there is a large area of peeling paint on the wall in the Busybee's classroom. Plan of Correction: We will contact the landlord about the repairs and remind teachers to be extra careful with children around those areas until they are fixed.
Standard #: 22VAC40-185-290-3 Description: Based on inspection, all electrical outlets did not have protective covers. Evidence: 2 outlets in the School Age classroom did not have protective covers. Plan of Correction: We will replace the covers and remind teachers to make sure outlets are covered at all times.
Standard #: 22VAC40-185-510-N Description: Based on medication review, medications were kept on site with an expired written authorization form. Evidence: The written authorization form for Child #1 had an expiration date of 8/20/2017. The parent was not notified to pick up the medication (the medication had not expired) or renew the authorization. Plan of Correction: We will contact the parent immediately and send the medication home with a request for an updated written authorization form.
