Comments:
The licensing inspector conducted an unannounced monitoring inspection from 9:15am - 1:00pm. There were twenty four children in care ages 2 years - 5 years with five staff supervising. The children were observed at free play, at story time, coloring, during lunch and during naps. Five staff's records and five children's records were reviewed. Violations from the previous inspection were also reviewed and all had been corrected. Compliance for standard 330-B could not be determined today because the ground is frozen and most of the playground is covered with snow. Standard #: 22VAC40-185-160-C
Description: Based on record reviews, the center failed to obtain a repeat tuberculosis screening every 2 years. Evidence: 1. The inspector reviewed five staff's records. One of five records reviewed (staff record #4) contained a tuberculosis screening that was over 2 years old. The screening was dated 08/06/015.

Plan of Correction: Staff #4 will obtain a repeat tuberculosis screening this week.

Standard #: 22VAC40-185-240-A
Description: Based on record reviews, the center failed to document orientation training for a staff. Evidence: 1. The inspector reviewed five staff's records. One of five records reviewed (staff record #1) did not contain documentation that staff had received the required training by the end of staff's first day of employment.

Plan of Correction: The director said staff#1 did receive orientation, but the documentation has been misplaced. The director will locate the orientation document and put it in the staff's record.
