A supplemental page was provided.
A Subsidy Health and Safety inspection was conducted on December 11, 2017 to review supplemental health and safety requirements. The on-site inspection was from 9:40 a.m. to 2:00 p.m. On this date, there were 22 children present and in the care of 5 staff members. There were two additional staff members available to assist if needed. There were 32 children enrolled in the center. Seven staff members' records and five children's records were reviewed and four classrooms were observed. There was no medication on-site. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection requirements and is identified on the violation notice.
Standard #: VENDSUB-000-006-B-11 Description: Based on review of the record for one child with a diagnosed food allergy, the center failed to obtain a written allergy care plan for the child. Evidence: 1. The record of Child A did not contain a written allergy care plan from a physician for the child's diagnosed food allergy. 2. The director confirmed the child did not have an allergy care plan from a physician on file. Plan of Correction: The director called the parent of the child during the inspection, and will obtain the written plan from the physician as soon as possible.
