Discussion was held on the topics of current enrollment, current staffing, and the new definition of a serious injury.
The following inspection took place between the hours of 2 pm through 4 pm. Today, I met with the Staff of the County After School Activities (CASA) program located on site at Legacy Elementary school in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 61 children with 4 staff; good ratios for this one group. A total of 6 children?s files were reviewed. Staff files were reviewed on an earlier date. The children were engaged in the following: Check-in, bathroom breaks, hand washing, snack, homework, art, games, large motor activities in the gym, and other organized activities. Standard #: 63.2-1701-G Description: Based on observation, the current license was not posted. Evidence - 1. On the date of inspection (12/13/2017), the current license, located on the site bulletin board reflected the expiration date of 05/06/2016. This is not the current license. 2. The current license would have an expiration date of 05/06/2018. Plan of Correction: The current license shall be located and posted. If it can not be located then a duplicate license shall be requested.
