Thank you for your assistance during this unannounced monitoring inspection. This on-site inspection was conducted on December 11, 2017 from 11:00 a.m. through 2:30 p.m. Upon arrival, there were eighty-seven (87) children in the care of seventeen (17) staff. Nine (9)) children's and nine (9) staff records were reviewed and updated. The medication and medication paperwork was reviewed for two children. Areas of non-compliance may be found on the violation notice. Areas observed were: outside play, hand washing/bathroom and diapering procedures, lunch, infant feeding and nap. Interactions between children and staff were observed as positive and the children seem comfortable with the daily routine and schedule. Standard #: 63.2-1721.1-B-3 Description: Based on a review of staff records, the center had not requested a state child abuse/neglect search within 30 days of hire for one staff. Evidence: 1. Nine staff records and two administrative records were reviewed on December 11, 2017. 2. One staff record did not have documentation of requesting an out of state central registry check within 30 days of hire. 3. Staff #3's date of hire was documented on 08/08/2017. 4. The director confirmed the hire dates for the three employees. Plan of Correction: Out of state background checks for a staff member hired after July 1, 2017 had not been completed. Upon notification, December 11, 2017, background check request for Michigan and Washington were submitted. Moving forward, Sworn Disclosures will be reviewed carefully to ensure any notation of out of state residency will trigger an immediate submission of a background check request.
