Standard #: 22VAC40-185-130-B Description: Based on record review, documentation of updated immunizations every six months for children under the age of two were not obtained. Evidence: Child #1 date of birth is 6/20/2016 and the most recent documentation of immunizations is dated 1/6/17. Plan of Correction: I'll email the parent and ask them to bring me a copy.
Standard #: 22VAC40-185-50-A Description: Based on observation, children's records were not treated confidentially. Evidence: A list of children's food allergies and preferences including the children's pictures, first and last name was posted in the Infant B classroom. This information was not covered. Plan of Correction: I'll print another cover and we'll get it posted.

Evidence: There are various cleaning and sanitizing materials stored on the same shelf with children's sippy cups under the counter in the Infant A classroom. Plan of Correction: We'll move the cleaning materials.

Evidence: Child #7 was observed laying on their cot with a bottom sheet during nap time. The child did not have a top sheet/blanket. Plan of Correction: We have extra blankets but the child never wants one.
