Comments:
An unannounced monitoring inspection was conducted this date. A total of 17 children with 3 staff on site at the beginning of the inspection. Children were observed finishing snack, sitting in small groups, completing homework, making an art project out of coffee filters and coloring with colored markers. Line up and transition from the cafeteria back to the gym area was observed. Staff strategically placed in both front and end of line to ensure supervision of children was ensured. No medications at this time. MAT/CPR and First Aid/daily health training all verified. A total of 5 children's records were reviewed. Note: Staff files are reviewed on an annual basis by representatives from the Commonwealth of Virginia, Department of Social Services, Division of Licensing, Fairfax Licensing Office and through the Loudoun County YMCA administrative offices located in either Reston and/or Leesburg, Virginia. Any observed standards found to be out of compliance would be cited in a violation notice issued at that time. This inspection began at 4pm and ended at 5:20pm. Thank you for your cooperation today. Standard #: 22VAC40-185-60-A
Description: Based on child record review, 4 out of 5 child records were incomplete. Evidence: Child #1' second emergency contact was missing an address. Child #2's record did not have addresses for the two emergency contacts. Child #3's second emergency contact was missing an address. Child #4 did not have a second emergency contact to include a phone number and address.

Plan of Correction: We will obtain this information from the parents this week and include it in their records.

Standard #: 22VAC40-185-550-D
Description: Based on review of the documented evacuation drill log, monthly fire drills have not been completed. Evidence: No documented evacuation drill was found for the month of November.

Plan of Correction: We will ensure that a monthly evacuation drill will be conducted.

Standard #: 63.2-1809-A
Description: Based on child record review, 2 out of 5 records did not have documentation of proof of identity. Evidence: Chid #2 and #5 did not have proof of identity documentation.

Plan of Correction: We will obtain the proof of identity from the children's parents
