63.2 Facilities and Programs..
- Technical Assistance:
An unannounced monitoring inspection was conducted on Monday, December 18, 2017 to determine the program?s compliance with licensing standards. The licensing inspector was on site from approximately 3:10 PM until 5:30 PM. Upon arrival, 58 school age children were being supervised by six staff members. Children were observed playing outside, working math problems, playing on the computers, drawing, watching the Grinch movie, and playing games. Staff was observed interacting with the children and providing assistance when needed. All areas used by children were inspected to include the main classrooms, hallways, bathrooms, cafeteria and playgrounds. The following information was reviewed during the inspection: injury reports, daily attendance, emergency drill logs, emergency supplies, transportation documents, field trip permission forms, parent agreements, liability insurance and menu. Last documented health inspection occurred on 02/13/2017 and the last documented fire inspection occurred on 03/02/2017. During the inspection, six children's records were reviewed. Staff records are stored off site and will be reviewed by licensing inspectors at a later date. Standard #: 22VAC40-185-60-A Description: Based on the review of six children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #3 contained one of the two required emergency contacts and information. Each child's record is required the name, address and phone number of two emergency contacts other than parent or guardian. Plan of Correction: We will have the family add the information to the child's record.
Standard #: 22VAC40-185-70-A Description: Based on documentation review, the required staff information was not present at the facility. Evidence: During the inspection, the licensing inspector reviewed information located at the facility for 12 staff members. The information present at the facility included a name and phone number of an emergency contact but not the physical address. for 11 of the staff members. Plan of Correction: We will request the information and add it to the staff binder.
Standard #: 22VAC40-185-540-C Description: Based on observation, the facility did not ensure all required items were located in first aid kit. Evidence: During the inspection, the licensing inspector checked the facility's first aid kit. The kit did not contain at least two triangular bandages. This was confirmed by members of management. Plan of Correction: We will add an additional triangular bandage to the first aid kit.
