Standard #: 22VAC40-185-60-A Description: Based on the review of six children's records, one record did not contain the required information. Evidence: During the inspection, the licensing inspector observed the record for Child #3 contained one of the two required emergency contacts and information. Each child's record is required the name, address and phone number of two emergency contacts other than parent or guardian. Plan of Correction: We will have the family add the information to the child's record.
Standard #: 22VAC40-185-70-A Description: Based on documentation review, the required staff information was not present at the facility. Evidence: During the inspection, the licensing inspector reviewed information located at the facility for 12 staff members. The information present at the facility included a name and phone number of an emergency contact but not the physical address. for 11 of the staff members. Plan of Correction: We will request the information and add it to the staff binder.
Standard #: 22VAC40-185-540-C Description: Based on observation, the facility did not ensure all required items were located in first aid kit. Evidence: During the inspection, the licensing inspector checked the facility's first aid kit. The kit did not contain at least two triangular bandages. This was confirmed by members of management. Plan of Correction: We will add an additional triangular bandage to the first aid kit.
