Technical assistance was provided in the following areas of the vendor agreement: VENDSUB-000-(5)-009.2 (Seroius injuries); VENDSUB-000-(9)-32.A.3 (Lockdown drills)
A Subsidy Health and Safety inspection was conducted on 12/18/17 to review supplemental health and safety requirements. The Licensing Inspector arrived at 10:20am and departed at 1:30pm. At the time of the tour, there were 55 children, ages six months old through four years old. Children were observed participating in various class activities, playing outside, eating lunch, and during rest period. The sample size was five children and six staff. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations were found during the supplemental subsidy health and safety inspection requirements.
Standard #: VENDSUB-000-031-A Description: Based on a review of facility's emergency preparedness plan and interviews, it was determined that the vendor failed to ensure that the evacuation procedures included procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child. Evidence: 1. The emergency preparedness plan for the facility did not include procedures for reuniting children with their parents or guardians for all types of emergencies. 2. Staff #7 (Assistant Director) confirmed that the procedure for how they would reunite children and parents/guardians was not in the emergency preparedness plan, but they are currently working on updating the facility's emergency preparedness plan. Plan of Correction: The vendor responded: We will add the reunification procedures how we would reunite children with their parents/guardians to to the emergency preparedness plan
