Comments:
An unannounced monitoring inspection was conducted from 9:30 am - 11:30 am. There were 37 children in care being supervised by 7 staff members. The children were observed participating in circle time activities, structured center play, and table top activities. Lunch is provided from home. Ten children's records were reviewed and in compliance. Five staff records were reviewed. Five medications were reviewed. There are currently two staff members on site who are MAT (Medication Administration Training) certified. The most recent emergency escape drill was conducted 12/1117 and the most recent shelter in place drill was conducted 04/20/2017. The first aid kit, flashlight, and radio were observed and in compliance. Standard #: 22VAC40-185-160-C
Description: Based on review of five staff records it was determined that not all records contained results of a TB (Tuberculosis) screening every two years. Evidence: The most recent TB screening on file for Staff A was dated 08/29/2015.

Plan of Correction: Administration will request an up to date TB screening for Staff A.

Standard #: 22VAC40-185-510-A
Description: Based on observation of five medications it was determined that not all policies in regards to medication were followed. Evidence: The emergency medication on site for Child A did not accompany a parental authorization.

Plan of Correction: Administration will request that the parents of child A complete a medication authroziation form.
