Comments:
An unannounced monitoring inspection was conducted from 11:30 am - 12:45 pm. There were 14 children in care being supervised by two staff members. The children were observed participating in circle time and singing activities. Snacks are provided from home as this is a half day program. Five children's records were reviewed and in compliance. Two staff records were reviewed and in compliance. One medication was reviewed and in compliance. There are two staff on site who are MAT (Medication Administration Training) certified. Standard #: 22VAC40-185-290-3
Description: Based on observation of the four's classroom it was determined that not all outlets were secured with protective covers. Evidence: 1. The electrical outlet between the closet and the boys bathroom did not have protective covers. 2. There was an open electrical box with wires exposed that did not have a cover.

Plan of Correction: 1. Administration will insert outlet covers in any empty electrical outlets. 2. Administration will install a temporary cover until a permanent cover can be installed.
