Comments:
An unannounced monitoring inspection was conducted from 10:25 am - 12:25 pm. There were 33 children present with five staff actively supervising. The children were engaged in seasonal art projects, center play, and eating snack. Six medications for three children were observed and in compliance. Three children's records and two staff records were reviewed - please see Supplemental Summary for missing item. The last health inspection was conducted 6/20/17. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of three staff records, the facility did not have documentation of two or more references as to character and reputation as well as competency before employment for all staff. Evidence: Staff A, (date of hire 9/17/15), did not have documentation of two reference checks.

Plan of Correction: The facility will obtain a second reference for Staff A.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the facility failed to sanitize the tables as required. To sanitize is to " treat in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution." Evidence: the facility used Clorox Clean Up wipes to wipe down the table after snack today. The sanitizer must be sprayed on the table.

Plan of Correction: Facility staff will use a bleach water solution or commercial sanitizer to sanitize the tables before and after meals.
