Comments:
An unannounced monitoring inspection was conducted today December 18, 2017 to determine compliance with licensing standards. The inspection was initiated at 10:00am and concluded at 11:45 am. At the time of the inspector's arrival children were entering the cafeteria for lunch. Thirteen children and three staff were in attendance. Staff sat with children and assisted with opening the top of the fruit cups and milk. Today's lunch consisted of chicken tenders, peaches, potatoes and milk. Children were excited to see the chicken tenders were shaped like Christmas trees. Upon returning to the classroom children gathered for a story before working in centers (tablets, house keeping, counting bears and blocks). Later children were participating in the "winter Olympics" program where they play games focused in areas of math and literacy. Today's game was math bingo. Staff were observed circulating throughout the classroom participating in activities. The classroom is equipped with age appropriate materials and supplies. Children play on the grass and blacktop area when outside. The first aid kit and non-medical emergency supplies were inspected and found complete. All required information was posted on the parent bulletin board to include the License, last inspection notice, emergency numbers, daily schedule, evacuation maps, allergy list and menu. The inspector interacted with staff and children in care. Three staff records and five children's records were reviewed. Medications and medication authorizations were reviewed. Staff on site have current MAT certification, CPR, First aid and daily health observation training. The most recent evacuation drill was documented on 11/27/2017. One of the two required shelter-in-place drills was conducted on 10/19/2017. A second drill will need to be completed by the end of the school year. Standard #: 22VAC40-185-510-N
Description: Based on review of medication and authorization documentation, the facility did not ensure parents were notified that the medication was to be picked up within fourteen days or that the parent must renew the authorization. Evidence: The medication authorization for child #5 expired on 10/02/2017 however the medication was still on site.

Plan of Correction: Per the Staff: I will have the office to send me a medication authorization form and I'll send it home for mom to have the doctor complete.
