Comments:
An unannounced monitoring inspection was conducted on December 11, 2017 from approximately 1:00 pm to 2:40 pm. Three staff were present with seventeen children. Children were observed eating lunch and napping. Five children's records were reviewed. Staff training was reviewed for three staff.

Description: Based on observation, facility did not ensure when pillows are used they are covered with pillow cases. Evidence: On December 11, 2017, Licensing Inspector observed three children napping on cots with pillows that were not covered with pillow cases.

Description: Based on review, facility did not ensure 2 of 5 injury reports have documentation of required information. Evidence: On December 11, 2017, the following was reviewed: 1. An injury report completed on July 18, 2017 did not document the date parent was notified. 2. An injury report completed on August 10, 2017 did not document a second signature.
