An unannounced monitoring inspection was conducted from 1:50 - 4:00 pm. There were 62 children present today. All rooms were within ratio. I observed children playing outdoors, napping, eating snack and playing with manipulative toys. If you have any questions, please contact me at

Standard #: 22VAC40-185-260-A
Description: Based on a review of records today, there was no available documentation for a current fire inspection. Evidence: The last fire inspection had an expiration date of 10/17.

Plan of Correction: They will be contacted again tomorrow.

Standard #: 22VAC40-185-270-A
Description: Based on observation today, there were some areas of the center not maintained in a safe and operable condition. Evidence: 1. Two tricycles in use today had broken areas. One was missing a handlebar cover, posing an entrapment hazard. Another had a sharply broken seat and a pedal missing. 2. A sink in the hallway was observed to have badly rotted surfacing on the lower part of the vanity. (repeat violation) 3. Some uneven areas in the factory made resilient material under the large slide to pose a tripping hazard. (repeat violation, 4/17) 4. Some trees behind a fence in the playground have blown over and had areas dangling in the playground. 5. One children's toilet was not working. According to staff confirmation, it has not been operable for two weeks. 6. The laminate floor around the floor drain was observed to be lifting where it is accessible to children. 7. In the 2 year old room, three children were observed to be sleeping with a blanket covering their head.

Plan of Correction: 1. These were taken out of the playground during the inspection, and parts will be ordered. 2. I will report it back to maintenance. 3. I will report it back to maintenance. 4. It has already been reported and they came to look at it yesterday afternoon. We will make the area off limits for the children. 5. I will call it in again. 6. I will report it back to maintenance. 7. I will talk to the teachers to make sure this does not occur.

Description: Based on staff confirmation and observation during today's inspection, craft materials not age appropriate were used with children under age 3. Evidence: Displays at child height and above included tiny pom poms and "googly" eyes which present a choking hazard.

Plan of Correction: We will have staff use age appropriate materials for eyes.
