Comments:
65 children were in care during the inspection. The age range of the children was three months to ten years old. Children were observed during rest time, free play, the school children went on a field trip to a farm. Five staff are current in MAT. Inspection 1:00pm-3:15pm

Violations:
Standard #: 22VAC40-185-240-D-1
Description: Based on review not all children's medications are given by staff trained in MAT. Evidence: Child A was given medication by a staff member that was not current MAT.

Plan of Correction: Only staff current in MAT will give medications.

Standard #: 22VAC40-185-330-B
Description: Based on observation the resilient surfacing was not six inches in the fall zones. Evidence: On the school age playground the slides did not have any mulch in the fall zones.

Plan of Correction: The slides will be off limits until the mulch is delivered.

Standard #: 22VAC40-185-510-A
Description: Based on review not all medication had current parent authorizations. Evidence: Child B's medication authorization is dated 5/26/2016

Plan of Correction: We will have the parents sign a new medication form.
