An unannounced renewal inspection was conducted with the assistant director beginning at 2:00 PM and 6:00 PM. An exit meeting was conducted with the director on December 21, 2017 beginning at 9:30 AM and ending at 10:15 AM. There were 168 children in care during the inspection. The children?s ages ranged between three months and five years old. The infants were observed playing with manipulatives, eating snack, and sleeping in cribs. The older children were observed during nap time and eating snack which consisted of celery, sun butter, and milk. The children were also observed playing with blocks and coloring. Sixteen children?s records and seven staff records were reviewed. Six medications were reviewed. The most recent fire inspection was documented as 9/20/2017.The most recent health inspection was documented as 10/3/2017. The most recent emergency escape drill was documented as 09/15/2017. The most recent shelter in place drill was documented as 05/18/2017. The first aid kit, flashlight, and weather band radio were observed and in compliance. The outside recreational areas were not inspected due to inclement weather.

Standard #: 22VAC40-185-130-A Description: Based on a review of documentation, the center did not obtain documentation of immunizations for all children prior to the first day of attendance. Evidence: Child A (date of enrollment 12/4/17) did not have documentation of immunizations. Plan of Correction: Proof of immunizations was requested and received by the center.
Standard #: 22VAC40-185-160-A Description: Based on review of seven staff records, it was determined that not all staff have documentation of a tuberculosis (TB) test or screening within 21 days of hire or within 12 months prior to hire. Evidence: Staff A?s (date of hire 10/2/17) TB test was dated 10/27/17. Plan of Correction: All new staff will have documentation of TB test or screening within 21 days of hire.
Standard #: 22VAC40-185-60-A Description: Based on a review of documentation, it was determined that the facility did not document viewing proof of identity for all children. Evidence: Child A, date of enrollment 12/4/17 did not have documentation of proof of age/identity. Plan of Correction: Proof of age/identity was requested and received by the center.
Standard #: 22VAC40-185-70-A Description: Based on review of documentation, the center did not maintain complete staff records. Evidence: Staff A (date of hire 10/2/17) and Staff B (date of hire 8/17/17) did not have documentation of two references. Plan of Correction: All new staff will have two references before date of hire.
Standard #: 22VAC40-185-240-A Description: Based on review of documentation, the center did not have documentation that staff received orientation training that includes job responsibilities, policies/procedures, playground safety procedures, child abuse and neglect, and confidentiality by the end of their first day of assuming job responsibilities. Evidence: Staff B (date of hire 8/17/17) did not have documentation of orientation. Plan of Correction: All new staff will have orientation by the end of their first day of employment.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all equipment inside the center was maintained in a clean and operable condition. Evidence: There were eleven rest mats in the science classroom found to have cracks making them absorbent. Plan of Correction: The request has been made to the corporate office to order new rest mats.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not keep hazardous substances stored in areas inaccessible to children. Evidence: Hair detangler was observed in an unlocked drawer in the art classroom; hand sanitizer was observed in an unlocked cabinet in the math classroom; Germ X was observed in an unlocked cabinet in the science classroom; alcohol was observed in an unlocked cabinet in the school age classroom. Plan of Correction: All hazardous substances will be kept in locked cabinets effective immediately.
