54 Children were incare during the inspection The age range of the children was four months to six years old. Children were observed during outside time and free play. Five children and six staff records were reviewed. Insoection time 2:00pm- 4:45pm
Standard #: 22VAC40-185-510-A Description: Based on review of children's medication not all had a parents authorization. Evidence: Child A's medication did not have a parents authorization. Plan of Correction: We will have the parent sign a medication authorization.
Standard #: 22VAC40-191-40-D-1-A Description: Based on review of five staff files not all had background checks with in 30 days of employment. Evidence: Staff A with a employment date of 9/11/17 did not have documentation of a search of central registry. Staff C did not have documentation of a sworn statement. Plan of Correction: We will resend all background checks for staff/
