Comments:
An unannounced monitoring inspection was conducted today beginning at 8:45am. At the beginning of the inspection the LI was informed the Director was on vacation and the Assistant Director was opening another school. The person who was third in charge was present (with all required qualifications). The Assistant Director arrived within 45 minutes. Classroom ratios were in compliance. The physical space, 6 staff records, 7 children's records, evacuation drills, the Emergency Preparedness plan, medications and attendance records were reviewed. Previous violations had been corrected and brought into compliance. Required documents were posted. Diapering and hand-washing procedures were observed and interactions between staff and children were positive. An abundant supply of age-appropriate materials were present. Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of physical space, the center was not maintained in a clean, safe and operable condition. Evidence: The toilets in the Early Preschool bathroom and the bathroom across from the Toddler's room had bolts protruding from the base that were not covered. In addition, at the beginning of the inspection several rooms were unoccupied but it was evident they had not been cleaned from the previous day. There was toilet paper on the floor, carpets had not been vacuumed and tables did not appear to have been wiped down. There was also dirt build up on the floors near baseboards and around the sink in the Early Preschool room.

Plan of Correction: We will put in a work order to fix the bolts and contact the corporate office about our cleaning company.
