Comments:
Today an unannounced Monitoring Inspection took place between the hours of 10:20am and 2:00pm. At the start of the inspection there were a total of 14 children with 6 staff; all ratios were incompliance. A sample of 4 children?s files and 5 staff files were reviewed. At the start of the inspection the children were observed playing freely in the gym. There is an ample supply of books and toys for the children. Today's lunch was fish sticks, rice, spinach salad, honeydew melon and milk. The center Director stated that there are no medications on site at this time. Standard #: 22VAC40-185-60-A
Description: Based on record review, children's files did not contain all required information. Evidence: Child #1 file did not contain and address for their first emergency contact. Child #3 file did not contain an address for their second emergency contact.

Plan of Correction: I will speak to the parents and have them provide me with this information.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, staff records did not contain all required information. Evidence: Staff #1 file did not contain the name, address or phone number of someone to be contacted in case of an emergency. The director confirmed that she had not yet asked this employee to provide this information.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-185-240-A
Description: Based on record review, documentation of orientation training was not available for one employee. Evidence: Staff #1 file did not contain documentation that orientation training had been provided by the end of their first day assuming job responsibilities.

Plan of Correction: This was completed but the paperwork was not signed. This was corrected during the inspection.

Standard #: 22VAC40-185-290-3
Description: Based on observation, unused electrical outlets did not have protective covers. Evidence: At the start of the inspection, all children were playing in a gym that had six unused electrical outlets that did not have protective covers.

Plan of Correction: I'll remind the staff to check for the outlet covers before the children use that space.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the most recent inspection of the facility was not posted. Evidence: The most recent inspection was conducted on 7/26/2017, however the current inspection notice that is posted is dated 4/20/2017.

Plan of Correction: Today's inspection will get posted.
