Comments:
An unannounced Monitoring Inspection was conducted on December 13, 2017 from approximately 11:50 a.m. until 3:15 p.m. Upon entrance there were 63 children and 8 staff present with children. Additional staff were on site for administrative purposes. The center's total enrollment is 70 children. The record review sample consisted of 6 children's records and 6 staff records. Children were observed during their rest period and infants were observed freely playing. Four medications were observed. The results of the inspection were reviewed with the center director. Violations:
Standard #: 22VAC40-185-450-A
Description: Based observation and staff interviews, the licensee did not ensure cots, used by children other than infants during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. Evidence: During the inspection on December 13, 2017, 15 preschoolers were observed during their nap time and 5 did not have a bottom cover and 1 did not have a top cover. There were 19 toddlers observed during their nap time and 3 did not have a top cover.

Plan of Correction: The program director will make sure the children will have a top and bottom or something large enough to fold.

Standard #: 22VAC40-191-60-B
Description: Based on documentation review and staff interviews, the licensee did not ensure all staff had a completed a sworn statement or affirmation before employment begins. Evidence; During the inspection on December 13, 2017, the record for staff #4 did not include an updated sworn statement with the question have you lived outside Virginia in the past 5 years?. The date of employment for staff #4 was 10-31-17.

Plan of Correction: The program director will have staff fill it out today.

Standard #: 22VAC40-191-60-C-2
Description: Based on documentation review and staff interviews, the licensee did not ensure all staff had obtained an a central registry finding within 30 days of employment. Evidence; During the inspection on December 13, 2017, the record for staff #2 did not include a CPS check conducted by the center within 30 days of employment. The record for staff #2 date of hire 5-8-17 had a CPS check completed by another child day center dated 8-15-2016. This CPS check does not meet the requirement and therefore a new check is needed. Staff #2 was observed working during the inspection and employment has not been denied.

Plan of Correction: The program director will conduct a check and staff will be removed from classroom and will work in other areas.
