Comments:
An unannounced monitoring inspection was completed today 12/28/2017. There were 42 children in care on the day of the inspection with seven staff. The inspector arrived at 11:15 and departed the program at 2:00. There were five children's and eight staff's records reviewed during the inspection. A physical plant walk through was completed, and observations of the children and four medications were reviewed. Discussion: Staff can only be counted in staff to child ratio when they are actively supervising the children. Please review standard VAC-22-40-185-350 F-2 when moving children from one room to another to meet ratio.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure that all records were complete according to the requirements of the standard. Evidence: 1. Children # 1 and 5 did not have an annual update. Child # 1 most recent completed enrollment form was 9/28/2015 and child # 5 was 9/14/2016.

Plan of Correction: The administrator will have the parents to update all information.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure that all area and equipment of the center, inside and outside maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed a small brown chair that was broken. The bottom was no longer attached to the chair. 2. In the school age room the bordering portion of the floor was loose and posed a trip hazard. 3. In the preschool room the TV cords were accessible to the children in care and posed a hazard.

Plan of Correction: Staff and owners will work on fixing all issues.
