An unannounced monitoring inspection was conducted on Thursday, December 28, 2017, from approximately 9:00am until 1:00pm. Some, but not all, licensing standards were inspected. The following classroom ratios were observed: Infant I = six infants with two staff Infant II = three infants with one staff Toddler I = five children with one staff Toddler II = six children with two staff Early Preschool 1= two children with one staff Early Preschool 2 - six children with one staff Preschool I = eleven children with two staff Pre-K = nine children with two staff Children were observed in a variety of activities. Two classrooms were making new year celebration items. Children were also building with magnet blocks and legos, and lacing cards. The younger children were receiving individual attention and having floor play with various manipulatives. Today, an individual from the library came to read a book to the children. Breakfast: pancakes, apricots and milk Lunch: turkey and cheese sandwich, green beans, mixed fruit and milk A practice evacuation drill was conducted 12/26/17. Health Inspection: 8/1/17 Fire Inspection: 5/25/17 Five children's records, one medication and five staff records were inspected.
Standard #: 22VAC40-185-140-A Description: Based on review of children's records, the center did not ensure each child's record contained documentation of a physical within 30 days of the child attending the center. Evidence: Out of five children's records inspected, Child #4, began attending the center 9/5/17, and does not have documentation of a physical. Plan of Correction: Per Center: Items missing in the child's record will be corrected. Management responsible for children's records will be retrained on file maintenance procedures.
Standard #: 22VAC40-185-40-G Description: Based on review of children's records, the center did not ensure each record contained proof of the child's identity within seven days of attendance.
Out of five children's records inspected, Child #2, began attending the center 7/13/17, does not contain documentation of viewing proof of identity.
Plan of Correction: Per Center: Items missing in the child's record will be corrected. Management responsible for children's records will be retrained on file maintenance procedures.
Standard #: 22VAC40-185-60-A Description: Based on review of children's records, the center did not ensure each record contained all required information. Evidence: Out of five children's records inspected, the records for Child #2, Child #3, Child #4 and Child #5 do not contain information regarding previous child care or a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease. Child #1 does not contain annual updated information. Plan of Correction: Per Center: Items missing in the child's record will be corrected. Management responsible for children's records will be retrained on file maintenance procedures.
Standard #: 22VAC40-185-270-A Description: Based on inspection of the facility, the center did not ensure all areas and equipment were maintained in a clean, safe and operable condition.
1. In the Preschool 1 classroom, the toilet paper container has broken free from the wall.
2. In the Preschool 1 classroom, the paint on the interior hallway entrance door has peeling paint.
3. In the Preschool 1 classroom, the red plastic couch is torn exposing the foam.
Plan of Correction: Per Center: Maintenance will be informed of the needed corrections.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records, the center did not ensure the results of a search of the central registry and any child abuse and neglect registry maintained by any other state in which the individual has resided in the preceding five years was documented in the staff record.
Out of five staff records inspected, Staff #5, began working at the center 1/9/17, but an out of state registry is not on record.
Plan of Correction: Per Center: The form will be mailed today and we will follow up with the state within 30 days if we have not received results. This is a new procedure that the center became aware of recently.
