Comments:
The Licensing Inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 12/19/17. Interviews were conducted with staff relating to the allegations of not writing an accident report. The information gathered during this investigation supports the allegations, so the complaint is determined to be valid. Description: Based on record review and interview, the center did not maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: An incident report was not written for a injury that was observed on 12/11/17. Staff #1 was interviewed and stated, on 12/11/17 the staff arrived to the crawler room at 7:30 am to relieve staff #2. Child #1 was sitting at the feeding table. Staff #1 reached over to take the hood off the infant, when doing so she noticed a red mark the size of a pencil eraser on child #1 neck. Staff #2 was interviewed and stated that around 7:45 am staff#1 came into my room and asked about the mark on child #1. Staff #2 stated that the infant arrived around 7:25 am and was placed in the feeding table. Staff #2 did have the opportunity to observe any mark on the infant. Staff #2 left the crawler room around 7:30 am when staff #1 arrived. Child #1 was observed this date 12/19/17, a faded mark was observed directly under the crease of the child's neck. The faded mark was about the size of a pencil eraser. The completed incident report was viewed this date, the injury was observed 12/11/17 and the report was signed by staff on 12/18/19.

Plan of Correction: The incident report was given to the parent the morning of 12/19/17 but the parent refused to take the report. I will write on the report "parent failed to sign" and give a copy to the parent this evening. In the future, accident reports will be completed the day of occurrence.
