22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 3:30pm-5:45pm. There were 110 children (infants-5yrs.) directly supervised by 27 staff. The physical plant, 10 staff records, 6 children?s records, 6 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, having music and spanish class, reading books, building with blocks and eating snack. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C Description: Based on review of three staff records, the center did not have documentation of a tuberculosis (TB) test or screening for staff at least every two years from date of the first initial screening or testing. Evidence: Staff C's most recent tuberculosis testing/screening was completed on 11/2/2015. Plan of Correction: Staff C's TB test and screening was completed on 12/20/17.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not maintain all equipment inside the center in a safe condition. Evidence: 1) In the Early Preschool Room, the play kitchen is missing pieces from the sink revealing holes which could cause a potential finger entrapment. 2) In the PreK 1 Room, the play kitchen is missing pieces from the sink revealing holes which could cause a potential finger entrapment. Plan of Correction: Play kitchen missing pieces were filled on 12/18/2017.
Standard #: 63.2-1720.1-B-3 Description: Based upon documentation reviewed, out of state child abuse and neglect results were not obtained. Evidence: Two of the ten staff records reviewed states that Staff A and B have lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained (Staff A; DOH 07/2017) (Staff B; DOH 08/2017). Plan of Correction: Documents completed on 12/18/17 and sent on 12/22/17.
