Comments:
An unannounced subsidy health and safety inspection was conducted with the assistant director beginning at 2:00 PM and 6:00 PM. There were 168 children in care during the inspection. The children?s ages ranged between three months and five years old. The infants were observed playing with manipulatives, eating snack, and sleeping in cribs. The older children were observed during nap time and eating snack which consisted of celery, sun butter, and milk. The children were also observed playing with blocks and coloring. Sixteen children?s records and seven staff records were reviewed. Six medications were reviewed. The most recent fire inspection was documented as 9/20/2017.The most recent health inspection was documented as 10/3/2017. The most recent emergency escape drill was documented as 09/15/2017. The most recent shelter in place drill was documented as 05/18/2017. The first aid kit, flashlight, and weather band radio were observed and in compliance. Standard #: VENDSUB-000-006-B-11
Description: Based on a review of documentation and staff interview, the facility did not have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child A and B did not have documentation of an allergy care plan in their records.

Plan of Correction: The facility will request the missing documentation from the parents.

Standard #: VENDSUB-000-012-E-1
Description: Based on a review of documentation, not all of the staff members had documentation of CPR certification appropriate to the age of the children in care. Evidence: Staff A's record did not have documentation of current CPR certification; the most recent CPR certification had expired on 10/11/17.

Plan of Correction: The facility will completes CPR training or provides documentation of current certification.

Standard #: VENDSUB-000-012-E-2
Description: Based on a review of documentation, not all of the staff members had documentation of first aid certification. Evidence: Staff A's record did not have documentation of current first aid certification, the most recent first aid certification had expired on 10/11/17.

Plan of Correction: The facility will completes first aid training or provides documentation of current certification.

Standard #: VENDSUB-000-031-A
Description: Based on a review of documentation and staff interview, the written emergency preparedness plan was not complete. Evidence: There was no documentation of a procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during emergency situations.

Plan of Correction: The facility will create or provide documentation of a reunification plan to add to the emergency preparedness plan and notify parents.

Standard #: VENDSUB-000-032-A
Description: Based on a review of documentation and staff interview, the facility did not complete all emergency response drills as required. Evidence: There was no documentation of a lockdown drill being practiced.

Plan of Correction: The facility will complete a lockdown drill and document it in the emergency escape drill log.
