An unannounced inspection was conducted on December 12, 2017 to investigate a complaint received by the licensing office on December 8, 2017. The complaint alleged noncompliance in the areas of supervision, food service, equipment maintenance, hand washing supplies, rest time, and notifications to parents. Observations were made and documentation was reviewed. The preponderance of the evidence obtained substantiated the complaint regarding supervision and equipment maintenance, and the complaint regarding those areas was determined to be valid.
Standard #: 22VAC40-185-270-A Complaint related: Yes Description: Based on observation, the equipment of the center was not maintained in a safe condition. Evidence: On the playground there was peeling paint on the slide structure near the top of the slide and on the top bar of the swing set. Plan of Correction: Working with maintenance team to crrect issues promptly.
Standard #: 22VAC40-185-350-E-2 Complaint related: Yes Description: Based on observation, the center did not ensure that for children 16 months to two years there was one staff member for every five children. Evidence: When the licensing inspection began at 8:25 a.m. on December 12, 2017 there were 10 toddlers (children 16 months to two years) in the Toddler Room. At 8:40 a.m. there were 12 toddlers with 2 staff. At 8:55 a.m. there were 13 toddlers with 3 staff. Plan of Correction: Schedule adjusted to being staff in earlier.
Standard #: 22VAC40-185-350-E-4 Complaint related: Yes Description: Based on observation, the center did not ensure that for children from three years to the age of eligibility to attend public school, five years by September 30 there was one staff member for every 10 children. Evidence: When the licensing inspection began at 8:25 a.m. on December 12, 2017 there were 20 children ages three years to five years in the Preschool Room. At 8:35 a.m. there were 23 preschool children with 2 staff. At 8:40 a.m. there were 24 preschool children with 2 staff. At 8:45 a.m. there were 22 preschool children with 2 staff. At 8:55 there were 26 preschool children with 3 staff. Plan of Correction: Schedule adjusted to bring in staff earlier.
