Comments:
An unannounced renewal study was conducted today from 9:20am-12:10pm. 21 children were directly supervised by 5 staff members. Classroom ratios were in compliance. The physical space, children's records, staff records, evacuation drills, medications, injury reports, emergency supplies, the Emergency Preparedness Plan, Transportation Policy, Playground Safety Policy and menu were reviewed. Required documents were posted. Previous violations were discussed and all violations had been brought into compliance. The space was clean and organized. An abundant supply of age-appropriate materials were available for the children. Cribs, diapering area, the cafeteria, rest area and parent information forms for infants were also reviewed. Children were observed in free play and small group activities. Interactions between staff and children were positive. The playground was covered with snow and unable to be inspected today. Standard #: 22VAC40-185-70-A
Description: Based on record review, 3 out of 3 staff records were incomplete. Evidence: Staff #1 and Staff #3 were missing documentation of contact for 2 or more references. Staff #2 was missing documentation of contact for 2 or more references and the Central Registry Search. Staff #2 was hired on 11/27/2017. Documentation was provided that the Central Registry Search was sent upon start of employment, but as of 12/27/2017 had not yet been received. No follow up was documented.

Plan of Correction: The references had been done, but were not documented, so we will correct our files. We will contact VDSS to see where the Central Registry Search is as far as the process goes.
