An unannounced complaint investigation was conducted to investigate a complaint related forbidden actions in the school age classroom at the center. The investigation was conducted with the director beginning at 9:50 AM and ending at 10:50 AM. One staff interview were conducted. One children?s record and two staff records were reviewed and in compliance. It was determined that the complaint was valid. Description: Based on staff interview, a staff used the forbidden action of demeaning verbal remarks. Evidence: Staff A stated that she made verbally demeaning remarks to a ten year old child on October 31, 2017.

Plan of Correction: The center will ensure that its staff is professional and will refrain from making demeaning verbal remarks to the children.
