Comments:
An unannounced monitoring inspection was conducted on January 9, 2018 at approximately 12:30pm - 4:45pm . The sample size of records reviewed included 5 staff records and 5 children's records. The children were observed napping, watching movie and parents picking up children. There were 35 children with 8 staff members and additional administrative staff were available. The exit interview was conducted with the program director.

Violations:
Standard #: 22VAC40-185-350-E-4
Description: Based on observation , the licensee did not ensure that for children from three years to the age of eligibility to attend public school: one staff member for every 10 children. Evidence: During the inspection conducted on January 9, 2018, the licensing inspector observed child #2( birth date 10/16/2013) in the school age classroom. There was one staff member with 12 children between the ages of four to six years old.

Plan of Correction: Today we had to move the children together because of emergency situation. Child #1 does attend the head start program in the public school and the director was unaware that she could not be counted as a school age child. The program director had extra staff in the room with the infants - three years of age that could of been moved to the school age classroom.

Standard #: 22VAC40-185-500-B
Description: Based on observation and staff interview, the licensee did not ensure there was a nonabsorbent surface for diapering . Evidence: During the inspection conducted on January 9, 2017, the licensing inspector observed a tear on the changing pad in the school age classroom making the surface absorbent. Staff #1 observed the tear on the diapering pad when licensing inspector was present.

Plan of Correction: The diapering surface used was for the emergency changing table. Children from ages infant to three year old were moved into the school age area due to the heating system issues. The diapering surface will be replaced.

Standard #: 22VAC40-185-510-E
Description: Based on record review, the licensee did not obtain medication in the original, labeled container Evidence: During the inspection conducted on January 09, 2018, child #1 medication was not in original container.

Plan of Correction: The program director has spoken to the parent about having the original container. The program director will talk to the parent again.
