Standard #: VENDSUB-000-006-B-11 Description: Based on record review of five children's records and interview, the facility did not ensure a child with diagnosed food allergies had a written allergy care plan that included instructions from the physician. Evidence: During the inspection, the licensing inspector observed the record for Child #2 did not contain a written allergy care plan that included all requirements. The record for Child #2 indicated the child had a food allergy. The record did not contain an allergy care plan that also included instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. A member of management also reviewed the records and the information could not be located during the inspection. Plan of Correction: We have previously requested an allergy plan from the family. We will again request this information from the family and add to the record.
