Comments:
Today an unannounced Monitoring Inspection took place between the hours of 12:00pm and 3:15pm. At the start of the inspection there were a total of 119 children with 13 staff; all ratios were incompliance. A total of 5 children?s files and 6 staff files were reviewed. The children were observed playing on the playground, working in small group centers and napping. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation of immunizations for one child before the child can attend the center. Evidence: Child #3 first date of attendance was 1/3/2018 and their file did not contain immunizations.

Plan of Correction: We will communicated verbally and via email of the form that we need.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all of the required information. Evidence: Child #1 file did not contain any emergency contacts to be reached in case of an emergency. Child #2, Child #3 and Child #4 file did not contain any address for their emergency contacts.

Plan of Correction: We've already created an updated application form that we will give to all of our families.

Standard #: 22VAC40-185-280-B
Description: Based on observation, cleaning materials were not kept in a locked place that prevents access by children. Evidence: Cleaning materials were observed in a cabinet under the sink in the Pre-K 2 B classroom. The keys to the cabinet were hanging in the lock and the cabinet was not locked, therefore making the materials accessible to the children.

Plan of Correction: We will have a conversation with these teachers about keeping cleaning materials locked up.

Standard #: 22VAC40-185-400-A
Description: Based on observation, one staff member did not promote a child's emotional well-being or interact with a child in a manner that provided needed help, comfort and support. Evidence: At the start of the inspection in the Pre-K B classroom, one staff member was observed telling a child "if you do not take a nap you're going home."

Plan of Correction: I will have a conversation with this staff regarding professionalism.

Standard #: 22VAC40-185-450-A
Description: Based on observation, not all children were provided with a top and bottom linen during their rest their designated rest period. Evidence: Child #6 did not have a top or bottom sheet to rest with during their nap. Child #7, Child #8 and Child #9 did not have top sheets to rest with during their nap.

Plan of Correction: We will send a family out to all of our families reminding them of naptime requirements.

Standard #: 22VAC40-185-500-A
Description: Based on observation, hand washing was not practiced as required. Evidence: During the inspection an infant was observed having their diaper changed in the Infant A classroom. The infants hands were not washed after their diaper was changed.

Plan of Correction: I will have a conversation with our entire infant team.

Standard #: 22VAC40-185-500-B
Description: Based on observation, there is not a non-absorbent changing mat in one classroom, as required. Evidence: There is a tear in the changing mat in the Beginner A classroom making the surface absorbent.

Plan of Correction: We have extra, we will replace it today.

Standard #: 22VAC40-185-560-G
Description: Based on observation, food that was brought from home was not labeled with the date and name to identify the owner. Evidence: There were three separate baggies filled with snacks on the counter of the Beginner B classroom. The baggies were not labeled with names or dates.

Plan of Correction: We will send out a staff memo making the employees responsible for labeling childrens food brought from home.
