22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted with the assistant director beginning at 12:35 PM and ending at 2:45 PM. There were 49 children, ages 5 months through 5 years, in care during the inspection. The infants were observed playing with manipulatives, sleeping in cribs, and being held by staff members. The toddlers, preschool, and junior kindergarten classes were observed sleeping on cots while soft music played. Ten children's records were reviewed. Five staff records were reviewed and in compliance. There were no medications on site to be reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The outside recreation areas were not inspected due to inclement weather. The most recent emergency escape drill was documented as 12/27/2017. The most recent shelter in place drill was documented as 1/23/2017. The most recent lockdown drill was documented as 7/31/2017. The most recent health inspection was documented as 10/06/2017. Standard #: 22VAC40-185-130-A
Description: Based on a review of documentation, the center did not obtain documentation of immunizations for all children attending the center. Evidence: Child A, with a start date of January 8, 2018, did not have documentation of immunizations in their record.

Plan of Correction: The center will request the missing documentation from the parent and place it in the record upon receipt.

Standard #: 22VAC40-185-570-E
Description: Based on observation, the center did not ensure that all prepared infant formula was dated and labeled with the child's name. Evidence: There were three bottles of prepared formula in the infant room's refrigerator that were not dated.

Plan of Correction: The bottles were dated during the inspection. The center will ensure that all bottles are check that the parents have labeled and dated all the bottles before placing them into the refrigerator.
